As a UI/UX designer, I was tasked with redesigning the user interface to reflect our client's new branding while making the form engaging and user-friendly.

To achieve this, I worked closely with our clients to understand their user flow and used feedback from fellow doctors to translate it into a UI design that is both visually appealing and easy to use. The challenge was to take a long, sometimes tedious form and make it engaging and efficient.

I accomplished this by using dynamic fields that are only triggered when required, collapsing non-relevant sections for specific patients, and carefully designing fields to keep them tight. The result is a form that's not only easy to use, but also saves doctors time while improving patient outcomes.
