Doctors were filling in what they didn't need to reach what they did.
I rebuilt the patient form for eye care specialists across New Zealand — optometrists, opticians, ophthalmologists. Conditional logic surfaced only what each patient required. Non-relevant sections collapsed by default.
A form that adapted to the patient, not the other way around.
